Please use the form below to request a copy of your Baptism, Confirmation or a Freedom to Marry Certificate (This also includes your Baptism and Confirmation information if we hold them.)

There is a set fee for Certificates which is £10 or £15 if you wish these to be posted to your home address. These fees support the Parish Financially. Please click the PayPal link at the bottom of this form to complete the fee after submitting the form request.

Fees should be paid by using the PayPal form below, PayPal will accept all major Credit and Debit Cards.

Please ensure you include your postal address if your form is to be posted to you. If you are collecting the form please be aware these will take 2 working days to complete. The forms can be collected Tuesday to Thursday between 10am – 2pm or by arrangement outside of these times.

Please note certificates cannot be emailed as they must contain the Parish Seal to be valid.
